Job summary

Integrity Care Group is seeking an Advanced Practice Provider (NP/PA) to deliver high-quality care for elderly residents in a long-term care facility. You will work with physicians, nursing staff, and other clinicians to manage chronic conditions and ensure resident well-being.

Responsibilities include rounds, documenting in the EMR, coordinating labs and imaging, and communicating orders to nursing staff. This role involves after-hours contact and safety-sensitive duties.

Qualifications

• Current Nurse Practitioner or Physician Assistant license.
• Board certification Nurse Practitioner or Physician Assistant.
• State and federal licensure including DEA.
• BLS certification.
• Knowledge of HIPAA, OSHA and related regulations.

Responsibilities

• Perform rounds in the facility, obtain history, exam, and document findings in the EMR.
• Organize rounding schedule, notes, labs and imaging reports in the EHR.
• Communicate with Director of Nursing about rounds and relay new orders promptly.
• Be the first point of contact for nurses with questions; occasional after-hours contact.
• Notate findings and treatment course with follow-up notes in the patient chart.
• Manage special projects and duties as assigned; this is a safety-sensitive role.

Position Summary:

Xpress Wellness Urgent Care is seeking a skilled and compassionate Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our dedicated team to provide high-quality care to elderly and medically complex residents a long-term care facility. The Advanced Practice Provider (PA/NP) will work collaboratively with physicians, nursing staff, and other healthcare professionals to provide comprehensive care, manage chronic conditions, and ensure the overall well‑well-being of residents.

Duties and Responsibilities:

• Perform rounds in the facility to include obtaining a history from the patient and supplemental sources, performing an exam and documenting findings within an electronic medical record.
• Organize a rounding schedule, notes, labs and imaging reports in the EHR for patients seen within the practice.
• Communicate with the Director of Nursing or floor nurses before and after rounds to obtain his/her concerns and to relay any new orders to them. New orders should be written at the time of rounds in order for those orders to become activated.
• The Provider will be the first point of contact for the nurses if questions arise on patients seen. This will require occasional after hours calls and/or texts.
• Notate findings and treatment course in patient chart including follow-up notes.
• Manages special projects and duties as assigned. This is a safety-sensitive position.

Requirements

• Current Nurse Practitioner or Physician Assistant license
• Board certification Nurse Practitioner or Physician Assistant.
• State and federal licensure requirements including DEA.
• BLS certification
• Knowledge of state and federal regulations including OSHA, HIPAA, blood borne pathogens and others.
• Excellent communication and interpersonal skills
• Ability to work efficiently and effectively in a fast‑paced and high-stress environment
• Knowledge and experience with electronic medical record systems
• Understanding of medical coding and billing.
• Strong decision-making and problem-solving skills
• Ability to work collaboratively with other healthcare professionals and staff.
• Competent with common PC applications including Internet, Email and Microsoft Office.
