Job summary

Citizens Care & Rehabilitation Center is seeking an Activities Assistant to plan and implement engaging activities for residents, enhancing their mental, physical, spiritual and social well-being. You will motivate participation, plan calendars, and document assessments and care plans while coordinating with nursing staff.

Responsibilities include transporting residents to programs, attending trainings, and maintaining clear written and verbal communication in English.

Qualifications

• High School Graduate or GED equivalent.
• One year experience in a recreation setting preferred.
• Must communicate effectively in English (oral and written).

Responsibilities

• Assist in development and implementation of activities.
• Motivate residents to participate daily.
• Plan individual community calendars and write individualized care plans.
• Complete assessments and maintain resident forms and records.
• Transport residents to and from programs; communicate with nursing staff.
