Job summary

Visions Case Management & Consulting in New Mexico seeks a capable service provider to strengthen communities by supporting clients and collaborating with staff.

You will maintain client files, draft annual plans, update Google Sheets, and participate in monthly training while traveling within the state as needed. Flexible hours and hybrid work are offered to empower the team.

Qualifications

• Four year degree in Nursing, Psychology, Social Work or related field with at least 1 year of related experience.
• Alternative: any four year degree with at least 6 years of related experience.
• Must be willing to travel throughout the state of New Mexico.

Responsibilities

• Facilitate monthly contact with consumers and submit related documentation in a timely manner.

• Maintain complete and current client case files.
• Utilize and update internal tracking systems (Google Sheets) 2x weekly.
• Participate in monthly staff meetings and supervision/training with regional lead or program manager.
• Respond to internal and external communications within 24 hours.

As a Service Provider at Visions Case Management & Consulting, you will be strengthening the communities of New Mexico by enhancing the lives of those we serve and work with. Will also be a source of information, to foster a supportive environment, to deliver exceptional services, and to empower everyone you interact with.

Responsibilities:

You will have the following responsibilities:

• Facilitate monthly contact with consumers and submit related documentation in a timely manner.
• Drafting and submitting annual plans/budgets according to program timelines and revise as needed.
• Maintain complete and current client case file.
• Utilize and update internal tracking systems 2 x weekly and ensure information is current and complete. Specifically google sheets or spreadsheets in general
• Participate in monthly staff meetings and either meet with your regional lead weekly or program manager monthly for supervision and training.
• Respond to internal and external communications within 24 hours.

Required Qualifications:

• Four Year Degree in Nursing, Psychology, Social Work and 1 year of related experience
• Any Four Year Degree and 6 years of related experience
• Travel within an entire state of New Mexico
