Qualifications

• MD or DO required and board certification or board eligibility in Otolaryngology-Head and Neck Surgery.
• Completion of an accredited Otolaryngology-Head and Neck Surgery residency.
• Eligible for Oregon medical licensure and DEA registration.
• Eligible for hospital privileges and payer credentialing.

Responsibilities
