Job summary

South Alabama Regional Planning Commission is seeking a Gateway Transition Coordinator to manage the initial response to referrals, complete the Return to Community Assessment, provide Options Counseling, and initiate the Individual Transition Plan under the Alabama HCBS program within the SARPC area. This role requires collaboration with discharge planners, waiver staff, and local ombudsman.

The candidate will ensure GCL informed consent is obtained, monitor ITP timelines, and perform outreach

Qualifications

• Bachelor’s Degree required in Social Work, Behavioral Sciences, Psychology, Geriatric Studies, or related field.
• At least one year of case management experience.
• Valid driver’s license with good driving record.

Responsibilities

• Serve as the first line for individuals transitioning from institutional care back to the community under ACT Waiver/GCL.
• Respond to referrals, complete RCA/Options Counseling, and obtain GCL Informed Consent.
• Initiate the ITP through a person-centered process and track implementation of the waiver Plan of Care.
• Collaborate with participant, discharge planner, and waiver case manager to monitor ITP timelines.
• Inform the GCL Office of projected move dates and ensure QoL Survey completion by the Ombudsman.
• Market ACT Waiver procedures and conduct monthly facility visit outreach.
• Ensure compliance with state and federal standards.
• Attend conferences and continuing education programs as required.
• Other duties as required.
