Qualifications

• Bachelor's degree in a human services field is required.
• Experience with intellectual disabilities or related fields is preferred.
• Strong communication and documentation skills are essential.

Responsibilities

• Provide consultative services to interdisciplinary teams.
• Assist in planning and implementing community living transition programs.
• Coordinate tours and liaise with community providers and LIDDAs.
• Maintain transition activity records and reports.
