Job summary

The South Alabama Regional Planning Commission seeks a Hospital to Home Transition Coordinator to support Medicaid clients moving from hospital to home. You will coordinate referrals, verify eligibility, and collaborate with hospital staff to ensure safe transitions and proper documentation.

Qualifications

Responsibilities

• Receives referrals and schedules assessments for transition.
• Verifies eligibility using available databases and records.
• Gathers information from hospital staff to determine program eligibility.
• Conducts prescreening with the client and collects missing documents.
• Submits case summaries to the H2H Nurse reviewer for viability.
• Refer ineligible or non-urgent cases to ADRC as appropriate.
