Job summary

Guardian Angel Senior Services, Inc. is seeking a Transitional Assistant Coordinator to support adults with disabilities and related clients in transitioning from skilled facilities to community living.

You will work under discharge planners and case managers to implement service plans that promote independence and safety. The role requires assisting with housing applications, setting up living arrangements, and maintaining ongoing communication with the case manager.

Qualifications

• Have a college degree (preferably in a human service field).
• Valid driver's license, clean driving record, and a reliable clean, insured vehicle; travel required.
• Maintain confidentiality of sensitive information.
• Utilize effective time management techniques to ensure tasks are prioritized and deadlines met.
• Complies with State/Federal regulatory acts and agency policies.
• Always respects and maintains client's confidentiality.

Responsibilities

• Assist with participant transition from facilities to community living with appropriate support.
• Assist housing search, voucher and housing applications; attend housing appointments.
• Set up apartments per participant preferences and coordinate utilities; maintain communication with case managers.
• Document services, housing applications, expenses and communications; track progress.
• Update services activity logs and submission of reimbursements in a timely fashion.

Education
