Job summary

The Western Reserve Hospital Physicians, Inc. seeks a Nurse Practitioner to perform comprehensive history and physicals across inpatient, outpatient, and ambulatory settings, and to contribute to an integrated management plan for general medical patients.

You will collaborate with an interdisciplinary team to ensure quality care and coordinate services with physicians, care managers, social workers, and other community agencies. Full-time position based in Hudson, Ohio.

Qualifications

• Master's degree or higher in Nursing.
• Active Ohio RN license.
• Ohio Prescriptive Authority certification.
• BLS required; ACLS preferred.
• Minimum 2 years of nursing experience in medical populations with leadership/management.

Responsibilities

• Perform comprehensive history and physical assessments in various settings.
• Develop an integrated management plan for general medical patients and communicate to the care team.
• Ensure quality of care using established standards and coordinate services with physicians, care managers, social workers and other teams.
