OnePeak Medical is seeking a skilled Medical Provider to join our growing team in Southern Oregon clinics (Talent, Medford, Central Point, Grants Pass, White City). The provider will deliver comprehensive medical services, foster lasting patient relationships, and collaborate with a high-functioning care team to achieve optimal health outcomes.

Qualifications

• Active Oregon license as a Certified Physician Assistant (or eligibility to obtain).
• Board certification in Family Medicine or Internal Medicine.
• Strong interest in preventive, functional, and lifestyle medicine.
• Excellent communication and patient relationship skills.
• Proficient in using electronic health records.

Responsibilities

• Provide direct patient care through office visits, telehealth, and occasional same-day appointments.
• Perform comprehensive assessments, order and interpret labs/imaging, and diagnose acute and chronic conditions.
• Develop personalized treatment plans that may include lifestyle interventions, supplements, hormone therapies, and prescriptions.
• Educate and empower patients on preventive health and functional medicine principles.
• Collaborate with internal health coaches, nutritionists, mental health providers, and clinical support staff.
• Document visits in the EHR in a timely and compliant manner.
• Participate in team meetings, quality improvement initiatives, and training sessions.
• Support clinic operations by contributing to a positive, team-oriented environment.
