OnePeak Medical is seeking a passionate Medical Provider (PA) to join our growing Southern Oregon clinics in Talent, Medford, Central Point, Grants Pass and White City. You will deliver comprehensive medical services, build lasting patient relationships, and collaborate with a high-functioning care team.

Qualifications

• Active Oregon license as a Certified Physician Assistant (or eligibility)
• Board certification in Family Medicine or Internal Medicine
• Strong interest in preventive, functional, and lifestyle medicine
• Excellent communication and patient relationship skills
• Proficient in using electronic health records
• Comfortable working in a fast-paced, collaborative environment

Responsibilities

• Provide direct patient care through office visits, telehealth, and occasional same-day appointments
• Perform comprehensive assessments, order and interpret labs/imaging, and diagnose acute and chronic conditions
• Develop personalized treatment plans that may include lifestyle interventions, supplements, hormone therapies, and prescriptions
• Educate and empower patients on preventive health and functional medicine principles
• Collaborate with internal health coaches, nutritionists, mental health providers, and clinical support staff
• Document visits in the EHR in a timely and compliant manner
• Participate in team meetings, quality improvement initiatives, and training sessions
• Support clinic operations by contributing to a positive, team-oriented environment
