Qualifications

• Active Oregon license as NP/PA/MD/DO or eligible.
• Board certification in Family or Internal Medicine or relevant specialty.
• Interest in preventive, functional, and lifestyle medicine.
• Excellent communication and patient relationship skills.
• Comfortable in fast-paced, collaborative environment.
• Proficient in using electronic health records.

Responsibilities

• Provide direct patient care via office visits and telehealth.
• Perform comprehensive assessments and order/interpret labs/imaging.
• Develop personalized treatment plans including lifestyle interventions and medications.
• Educate patients on preventive health and functional medicine principles.
• Collaborate with health coaches, nutritionists, mental health providers, and staff.
• Document visits promptly in the EHR.
• Participate in quality improvement and training sessions.
• Support clinic operations with a positive team environment.
