OnePeak Medical is seeking a passionate Medical Provider to join a forward-thinking team dedicated to whole-person care. You will deliver comprehensive medical services, build lasting patient relationships, and work with a high-functioning care team to achieve optimal health outcomes.

The role emphasizes a supportive environment, ongoing functional medicine training, and a 4-day work week with no nights or weekends, with relocation assistance available.

Qualifications

• Active Oregon license as NP/PA/MD/DO or eligibility to obtain
• Board certification in Family Medicine or applicable specialty
• Interest in preventive, functional and lifestyle medicine
• Excellent communication and patient relationship skills
• Comfortable in a fast-paced, collaborative environment
• Proficient with electronic health records (NextGen a plus)

Responsibilities

• Provide direct patient care via office visits and telehealth
• Perform assessments, order labs/imaging, diagnose conditions
• Develop personalized treatment plans including lifestyle, hormones, prescriptions
• Educate patients on preventive and functional medicine
• Collaborate with health coaches, nutritionists and staff
• Document visits in NextGen EHR timely and compliant
• Participate in team meetings and quality improvement
• Support clinic operations and teamwork
