Job summary

OnePeak Medical Providers is seeking a passionate Nurse Practitioner to deliver primary care with a focus on functional and preventive medicine in Oregon. You’ll work with a collaborative care team, provide direct patient care across office visits and telehealth, and develop personalized treatment plans.

Qualifications

• Active Oregon NP license (or eligibility)
• Board certification in Family Medicine or Internal Medicine
• Strong interest in preventive, functional, and lifestyle medicine
• Excellent communication and patient relationship skills
• Experience in fast-paced, collaborative environment
• Proficient in using electronic health records

Responsibilities

• Provide direct patient care through office visits, telehealth, and occasional same-day appointments
• Perform comprehensive assessments, order and interpret labs/imaging, and diagnose acute and chronic conditions
• Develop personalized treatment plans that may include lifestyle interventions, supplements, hormone therapies, and prescriptions
• Educate and empower patients on preventive health and functional medicine principles
• Collaborate with internal health coaches, nutritionists, mental health providers, and clinical support staff
• Document visits in the EHR in a timely and compliant manner
• Participate in team meetings, quality improvement initiatives, and training sessions
• Support clinic operations by contributing to a positive, team-oriented environment
