Job summary

Qualifications

• Active Nurse Practitioner license with full practice authority in Illinois
• National board certification (e.g., FNP, APRN)
• DEA registration
• Clinical experience in primary care/internal medicine/family practice
• Experience or interest in functional/integrative medicine
• Ability to order and administer IV therapy
• Strong assessment, diagnostic reasoning, and treatment planning skills
• Collaborative care with integrative practitioners

Responsibilities

• Conduct comprehensive patient assessments and review health histories
• Order and interpret diagnostic tests and develop individualized care plans
• Provide direct patient care including follow-up visits and medication management
• Coordinate services such as IV therapy with other clinicians
• Educate patients on lifestyle, nutrition and preventive strategies
• Document in the medical record and participate in care-team discussions
• Maintain current knowledge of functional medicine practices and regulatory standards
• Contribute to a patient-centered, collaborative practice culture
