Job summary

BAYADA Home Health Care, Inc. is seeking a compassionate Home Health Physical Therapist (PT) to deliver in-home care for Nevada, MO and surrounding areas. This full-time role focuses on reducing stress through patient-centered, at-home therapy and comprehensive home health services.

You will provide 1:1 care, participate in a multidisciplinary team, and utilize digital documentation. The role includes a retention bonus and a strong emphasis on training and professional growth.

Qualifications

• Active and current PT license.
• One year of clinical experience preferred.
• Travel locally to cases as assigned.

Responsibilities

• Visit clients in their own homes to aid recovery after injury, illness, surgery, or hospitalization.
• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements (mobility, range of joints, gait, strength, balance, safety).
• Implement wound care and use evidence-based dressings and topical agents.
• Develop and implement individualized care plans including exercises, gait training and balance.
• Revise care plans with nursing as part of the multidisciplinary team.
• Educate clients and families in rehabilitative care and independence.
• Document observations and interventions on a state-of-the-art touch pad tablet.

**$30,000.00 Retention Bonus Available** We are recruiting for a compassionate Home Health Physical Therapist (PT) to reliably serve patients in Nevada, MO and surrounding area. What you'll do:

Visit a variety of clients in their own home, helping them recover from an injury, illness, surgery, or hospitalization.

Responsibilities for a PT - Physical Therapist include:

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements, such as the mobility/range of joints, transfer status, stability, patterns and appearance of ambulation, strength and endurance of muscles, balance testing, and safety assessments.
• Implement integumentary protective interventions to prevent wound formation and promote wound repair utilizing evidence-based dressings and topical agents.
• Develop and implement appropriate individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
• Continually assess and revise the Physical Therapy - PT care plan, and participate with nursing in the multidisciplinary care plan, as appropriate.
• Educate and instruct clients, family members, or other client representatives, in rehabilitative care and activities necessary to promote the client's health, safety, and independent living.
• Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet.

What you should have:

• Active and current PT license
• One year of clinical experience preferred
• Previous home health care experience welcomed, but not required. We will train you!
• Ability to travel locally to cases as assigned
