Job summary

Riverstone Post Acute in Salina, KS is seeking compassionate RNs and LPNs for Day and NOC shifts on full-time or part-time basis. We welcome multi-state licenses and new graduates as well as seasoned nurses, offering competitive wages and flexible opportunities.

Positions require KS license, BLS/CPR, and a commitment to patient-centered care. Responsibilities include charting, narcotic counting, documenting condition changes, and collaborating with physicians, families, and the DON.

Qualifications

• Current LPN/RN Kansas license.
• Current BLS/CPR certification or willingness to acquire one.
• Experience in LTC/SNF is preferred but not required.

Responsibilities

• Follow charting and incident reporting procedures.
• Follow procedures for narcotic counting, pharmacy orders, physician orders.
• Document changes in patient conditions including nutrition, infections, skin issues, weight loss.
• Provide patient-centered care.
• Document and notify changes for room changes, LOA, admits or discharges.
• Rounds with physicians and communicate resident condition changes with Physician, family and DON.
• Full-time may require alternating weekends.
• Work with the team and maintain open communication.

• Documents any change in patient's conditions including but not limited to nutritional problems, infections, skin conditions, weight loss or any other noticeable changes.
• Understand Patient Centered Care
• Document and notify applicable departments of any patient's room changes, LOA, new admits, or discharges.
• Makes rounds with physicians when necessary and communicate resident condition changes with Physician, family and DON.
• Full time would require alternating weekend coverage.
• Work well with the team and have open communication.
