Job summary

Vantage Care Management is seeking a Full Time Speech-Language Pathologist to plan, organize, develop and direct Speech therapy services in a skilled nursing setting. You will assess residents, develop care plans, and ensure HIPAA compliance and high-quality patient care.

The role requires a Masters in Speech-Language Pathology and CCC or CFY eligibility with licensure in progress. You will collaborate with rehabilitation staff and discharge planning teams to optimize resident outcomes.

Qualifications

• Requires masters in Speech-Language Pathology from an accredited program.
• CCC or CFY eligibility and licensure eligibility required.

Responsibilities

• Plan, develop, organize, implement and evaluate Speech therapy services.
• Assess patients appropriate to age of the patient.
• Maintain accurate documentation and department records.
• Ensure effective communication regarding ST programs.
• Manage patient care and rehabilitation staff in daily operations.
• Comply with HIPAA guidelines and safety standards.
• Discharge planning and development of resident care plans.
• Complete evaluations and documentation timely per regulations.
