Job summary

ElderSpan Management is seeking a Full Time RN or LPN Nurse Care Coordinator in Baraboo, WI. The role centers on coordinating individualized care plans, guiding care teams, and ensuring residents receive clinical and compassionate support.

You will assess, plan, implement and evaluate resident care, collaborate with RNs, physicians and families, and maintain Plans of Care while updating the electronic health record, ensuring compliance with Wisconsin regulations.

Qualifications

• Registered Nurse License (RN) or Licensed Practical Nurse (LPN) in Wisconsin.
• Minimum 2 years of experience in long-term care settings.
• Nursing skills aligned with Wisconsin state regulations.

Responsibilities

• Assess, plan, evaluate and direct person-centered care for frail seniors.
• Oversee medication administration, training, injections and blood draws.
• Create and update each resident's Plan of Care and communicate changes promptly.
• Support the Administrator in their absence and collaborate with care staff.
• Document in the electronic health record completely and timely.

ElderSpan Management is a Wisconsin family-owned company and opened their first resident-centered, health-care focused assisted living facility in 1995.

We are seeking a Nurse to join the team!

Our Nurses play a vital role in enriching the lives of our residents by coordinating individualized care plans, guiding care teams, and ensuring each of our seniors receives the highest level of clinical and compassionate support. They are accountable for participating in assessment, planning, implementation and evaluation of resident care, as well as collaborating with the RN Administrators, care staff, physicians, and family members to ensure each resident maintains or attains the highest physical, mental and psychosocial well-being.

Shift: Other, See Hours Hours: Generally 9am-5pm, M-F Salary Range: $35/hr + Based on Experience

Responsibilities include (but are not limited to):

• Assess, plan, evaluate and direct person-centered care for frail seniors
• Oversee medication administration, training, injections and blood draws
• Create and update each resident's Plan of Care, communicating changes in condition to physician, family members and team members in a timely manner
• Assure that each resident's physical, mental and spiritual needs are met by working with and monitoring care staff and serving as assistant Administrator when delegated by the Administrator in their absence
• Document in the electronic health record in a complete and timely manner

What qualifications are we looking for?

• Registered Nurse License
• 2+ Years Experience in Long-term care
• Excellent nursing skills and in compliance with the State of Wisconsin regulations
