Fair Oaks Health Care Center in Crystal Lake, IL is seeking a clinically competent registered nurse to oversee care for residents using the nursing process. This on-site role requires collaboration with physicians and the health team to assess, plan, implement and evaluate patient care, and communicate changes effectively.

Responsibilities

• Provide safe, effective nursing care per care plans and physician orders.
• Administer medications and treatments accurately.
• Assess residents and report changes promptly.
• Document care in the EMR.
• Supervise and mentor LPNs and CNAs.
• Ensure infection control and safety compliance.
• Collaborate with the interdisciplinary care team.

Schedule:

Available Openings are: Full-Time (7p-7a - 3 days a week) Full or Part-Time (6a-noon). Part-time (3p-9p) or (3p-11p)

Job Summary:

A clinically competent, registered, professional nurse who assumes responsibility and accountability for a group of patients/residents through the nursing process of assessment, planning, intervention, implementation and evaluation. Effectively interacts with patients/residents, family members, responsible party and other health team members, while maintaining standards of professional nursing. The RN plans, supervises and evaluates care of each patient/resident. The RN ensures that the status of the patient/resident condition is effectively communicated to the physician.

Compensation:

• PLUS $2.00 per hour PM and $2.50 per hour NOC shift differentials
• $5.00 differential on weekend shifts if you work every other weekend a minimum of two shifts (or 16 hours)

Qualifications:

Required:

• Active RN license in state of Wisconsin or licensed in any U.S state and be eligible for Wisconsin licensure by reciprocity. (in good standing)
• CPR certified
• Ability to meet physical and job-related requirements
• Must be English literate and possess excellent written and verbal communication skills

Preferred:

• Long-term care or skilled nursing experience
• Supervisory or leadership experience
