Job summary

McLaren Health Care in Michigan is seeking a Registered Nurse Care Manager in Case Management to coordinate patient transitions and optimize length of stay, resource use, and readmission prevention. You will work with physicians, nursing, social work, Home Care, PCP offices, and other resources to ensure safe, appropriate transitions across the continuum.

Requirements include RN licensure, a Bachelor’s degree in nursing (or actively pursuing one with anticipated completion within five years),

Qualifications

• RN licensure (State license) is required.
• Bachelor’s degree in nursing or higher; pursuing degree acceptable with expected completion within five years.
• Three years of acute hospital care experience are required.

Responsibilities

• Perform care coordination assessments for initial admission within 24 hours.
• Collaborate with social workers and other disciplines to ensure timely transitions to appropriate levels of care.
• Assess patient/family needs to reduce barriers and plan discharge.
