Job summary

McLaren is seeking an RN Care Manager in Michigan to assess, plan, implement, coordinate, and evaluate services for patients, using clinical and community resources to promote quality, cost-effective outcomes. The role emphasizes evidence-based guidelines and patient-centered care to reduce hospitalizations and ER visits.

You will work with an interdisciplinary team, support self-management, coordinate transitions, and ensure documentation and communication across care settings.

Qualifications

• RN license must be valid and unrestricted in Michigan.
• Minimum 3 years nursing experience with chronically ill patients.
• Experience in care management and population health is preferred.

Responsibilities

• Provide telephonic and face-to-face comprehensive assessment and care management.
• Identify high-risk patients using screening tools and risk assessment.
• Plan, coordinate, monitor, and adjust individualized care plans.
• Coordinate transitions and post-discharge follow-up.
• Collaborate with PCP, interdisciplinary team, and community resources.
• Document care management activities and contribute to process improvements.
