Job summary

Gosnold, Inc. in Wareham, MA is hiring a Case Manager/CSS to support patients in recovery. The role starts in Falmouth and moves to Wareham this Fall, with shifts from 7a to 3p. A Bachelor's in healthcare and at least 1 year in mental health or related fields are preferred.

You will conduct assessments, develop treatment and discharge plans, facilitate groups, and document in the EHR while collaborating with referral sources and a multidisciplinary team.

Qualifications

• Bachelor's degree in healthcare or related discipline preferred.
• Minimum 1 year of experience in mental health, addiction, or home health/hospice.
• Excellent customer service and communication skills; computer literacy.

Responsibilities

• Meet with caseload daily to assess needs and progress.
• Conduct biopsychosocial and Columbia assessments within 48 hours.
• Prepare treatment and discharge plans with patients and supports.
• Facilitate groups as assigned and participate in supervision.
• Maintain documentation in Gosnold EHR per policy.
