Post Rehabilitation Services is hiring a full-time Physical Therapist Assistant for Maple Leaf in Manchester, NH. The PTA will provide skilled nursing rehabilitation under the Physical Therapist's direction, focusing on meaningful improvements in daily mobility.

Responsibilities include executing PT plans, guiding strengthening and balance activities, and ensuring safe mobility for residents. Competitive hourly pay and comprehensive benefits are offered.

Qualifications

• NH Physical Therapist Assistant license required.
• Strong communication, teamwork and clinical documentation skills.
• Ability to provide attentive treatment within an individualized care plan.
• New graduates welcome; SNF/post-acute experience is helpful.

Responsibilities

• Carry out individualized physical therapy treatments within the PT's plan of care.
• Coach residents through strengthening, balance and endurance activities.
• Assist with bed mobility, transfers and walking with appropriate assistive devices.
• Instruct and reinforce safe use of walkers and other mobility aids.
• Observe responses to treatment and report progress and barriers to the supervising PT.
• Document interventions accurately and collaborate with nursing/rehabilitation staff.
