Job summary

Creative Solutions In HealthCare in Texas seeks a full-time Physical Therapy Assistant (PTA) to join our in-house rehab team. You will provide patient-centered care to enhance mobility and quality of life under PT supervision.

The role requires an active Texas PTA license, familiarity with PT treatment practices, and experience in SNF/long-term care is preferred. Net Health EHR experience is a plus, with collaborative teamwork across disciplines.

Qualifications

• Active and valid Texas PTA license.
• In good standing with regulatory boards.
• SNF or long-term care experience preferred.
• Current knowledge of physical therapy treatment practices.
• Experience with Net Health EHR preferred.

Responsibilities

• Administer prescribed physical therapy treatments to relieve pain and improve function.
• Provide skilled PT services under supervision of a Physical Therapist.
• Respond to service requests and relay information to the PT.
• Implement and modify care plans under PT supervision.
• Measure and record motor function, strength, and performance.
• Monitor patient progress and adjust approaches with the team.
• Provide education to patients, families and caregivers.
• Ensure documentation complies with policies and regs.
• Attend meetings designated by the Director of Rehab.
