Qualifications

• Licensed Physical Therapist (PT) in the State of Ohio.

Responsibilities

• Evaluate and treat patients in a post-acute/skilled nursing setting.
• Develop individualized treatment plans focused on mobility, strength, balance and independence.
• Collaborate with Occupational Therapists, Speech-Language Pathologists, nursing staff, physicians and families.
• Complete accurate, timely documentation.
• Educate patients and caregivers on treatment plans and home exercise programs.
• Help patients achieve meaningful functional outcomes.
