Job summary

Healthcare Therapy Services (HTS) seeks a licensed Physical Therapist to join our therapy team at a CMS 5-Star continuing care retirement community on the northeast side of Fort Wayne, Indiana. This full-time role offers a varied caseload across short-term rehab, long-term care, outpatient, memory care, assisted living and independent living, with a well-equipped gym and strong interdisciplinary support.

Applicants must hold an Indiana PT license or be eligible; HTS emphasizes person-centered

Qualifications

• Active Indiana Physical Therapist license or eligibility to obtain one.
• Advanced degree from an ACPE-accredited physical therapist education program.
• Licensure exam completed and license requirements fulfilled in Indiana.

Responsibilities

• Deliver physical therapy services to residents across rehab, long-term care, memory care and outpatient settings.
• Collaborate with interdisciplinary team to plan and implement individualized care.
• Support patient-centered care and contribute to clinical excellence.

• Full-time, Monday–Friday, 30–40 hours/week.

Requirements

Applicants must have an active Indiana Physical Therapist license or must have the necessary qualifications to earn Indiana license.
