Qualifications

• Active Physical Therapist license in New Jersey or eligible to obtain.

• Experience in SNF/geriatrics is a plus.
• Strong communication and interpersonal skills.
• Ability to work collaboratively within an interdisciplinary team.

Responsibilities

• Evaluate and treat residents in subacute and long term care with orthopedic, neurological, and post-surgical conditions.
• Develop personalized treatment plans to improve strength, balance, gait, and mobility.
• Collaborate with physicians, nurses, PTAs, OTs, and SLPs.
• Document progress accurately and timely in the EMR.
• Participate in care plan meetings and family conferences.
