Country Arch Care Center is seeking a full-time Physical Therapist to join our rehabilitation team at a skilled nursing facility in New Jersey. You will evaluate and treat residents in subacute and long-term care, aiming to restore mobility and independence.

You will collaborate with physicians, nurses, OT, and SLP, develop personalized plans, document progress in EMR, and participate in care plan meetings. New grads welcome with mentorship.

Qualifications

• Licensed Physical Therapist in New Jersey or eligible.
• New grads welcome; mentorship and training provided.
• Experience in SNF, geriatrics is a plus.
• Strong communication and teamwork skills.
• Ability to document patient progress accurately.

Responsabilités

• Evaluate and treat residents in subacute and long-term care with orthopedic, neurological, and post-surgical conditions.
• Develop personalized treatment plans to improve strength, balance, gait, and mobility.
• Collaborate with physicians, nurses, PTAs, OTs, and SLPs.
• Document patient progress in EMR accurately and timely.
• Participate in care plan meetings and family conferences.
• Provide compassionate, patient-centered care in a team environment.
• Supervise PTAs as needed.
• Report to the Director of Rehabilitation.
