Job summary

We Care Health Centers in Ohio seeks a Nurse Practitioner to provide primary and preventive care to residents in long term care facilities. You will manage residents with acute changes, support advanced care planning and coordinate with an interdisciplinary team.

The role requires current OH NP license, board certification, DEA licensure, and experience in geriatric or adult care with EMR proficiency; sign-on bonus and robust benefits are offered.

Qualifications

• Current, unrestricted NP license in OH
• Board Certification as NP
• DEA licensure required
• Clinical background in adult, family or geriatric specialties
• Experience working in long-term care settings
• Ability to work with EMR
• Ability to manage patients with advanced illness and multiple chronic conditions
• Ability to work within an interdisciplinary care team

Responsibilities

• Provide primary and preventative care for long term care patients in skilled nursing facilities.
• Manage residents with acute changes in condition.
• Advanced care planning and chronic care management.
• Prescribe medications, including controlled substances, per OH law.
• Order, review and/or follow-up with treatments and diagnostic testing.
• Communicate with nursing home staff regularly and as needed.
• Collaborate with primary care physician.

We are currently seeking Nurse Practitioners provide medical services to patients living in long term care facilities.

NP ROLE AND RESPONSIBILITIES:

• Provide primary and preventative care for long term care patients residing in skilled nursing facilities.
• Manage Residents with acute change in condition.
• Advanced care planning and chronic care management.
• Prescribe appropriate medications, including controlled substances in accordance with OH Law.
• Order, review and/or follow-up with treatments and diagnostic testing.
• Communicate with the nursing home staff on a regular and as needed to discuss changes in condition.
• and/or the care plan.
• Collaborate with primary care physician.
