CareMed Health Solutions in Ohio is seeking a Nurse Practitioner to deliver primary and preventive care to residents living in skilled nursing facilities, with a focus on improving quality of life for elderly patients. You will manage acute changes, perform advanced care planning, order tests, and collaborate with primary care physicians within an interdisciplinary team.

Requirements include an Ohio NP license, board certification, DEA licensure, and experience in geriatric or adult primary

Qualifications

• Current, unrestricted NP license in Ohio.
• Board Certification required.
• DEA licensure required.
• Clinical background in adult, family or geriatric specialties.
• Experience in long-term care setting.
• Proficient with EMR systems.
• Ability to manage patients with complex, chronic conditions.
• Work within an interdisciplinary care team.
• Psych experience is a plus.

Responsabilités

• Provide primary and preventative care for long-term care residents in skilled nursing facilities.
• Manage residents with acute changes in condition.
• Perform advanced care planning and chronic care management.
• Prescribe medications in accordance with OH law, including controlled substances.
• Order, review and follow up with treatments and diagnostic testing.
• Communicate with nursing home staff about changes in condition and care plans.

While an interdisciplinary care team to provide medical care to patients in long term care facilities, you will have the opportunity to impact in a positive way the life and wellbeing of the elderly population.

We are currently seeking Nurse Practitioners provide medical services to patients living in long term care facilities.

NP ROLE AND RESPONSIBILITIES:

• Provide primary and preventative care for long term care patients residing in skilled nursing facilities.

• Manage Residents with acute change in condition.

• Advanced care planning and chronic care management.

• Prescribe appropriate medications, including controlled substances in accordance with OH Law.

• Order, review and/or follow-up with treatments and diagnostic testing.

• Communicate with the nursing home staff on a regular and as needed to discuss changes in condition.

• and/or the care plan.

• Collaborate with primary care physician.

NP QUALIFICATIONS AND EDUCATION REQUIREMENTS

• Current, unrestricted NP license in the state of Ohio.

• Board Certification.

• DEA licensure.

• Clinical background in adult, family or geriatric specialties.

• Experience working in a long-term care setting.

• Ability to work with an EMR.

• Ability to manage patients with advanced illness and multiple chronic conditions.

• Ability to work within an interdisciplinary care team to coordinate patient care.
