Job summary

We Care Health Centers is seeking a Nurse Practitioner to deliver primary and preventive care for residents in skilled nursing facilities. You will manage acute changes in condition, engage in advanced care planning, and coordinate with the facility nursing staff and a primary care physician to ensure comprehensive care.

Applicants must hold an OH NP license, Board Certification, and a DEA registration, with experience in long-term care and EMR systems.

Qualifications

• Current, unrestricted NP license in the state of OH.
• Board Certification.
• DEA licensure.
• Clinical background in adult, family or geriatric specialties.
• Experience working in a long-term care setting.
• Ability to work with an EMR.
• Ability to manage patients with advanced illness and multiple chronic conditions.
• Ability to work within an interdisciplinary care team to coordinate patient care.
• Psych experience a plus
