CareMed Health Solutions is seeking a Nurse Practitioner to provide primary and preventive care for residents in long-term care facilities. You will manage acute changes, perform care planning, and coordinate with the interdisciplinary team to optimize treatment plans.

Requirements include an Ohio NP license, board certification, DEA licensure, and experience in geriatric or family care settings. Long-term care and EMR experience are strongly preferred.

Formación

• Current, unrestricted NP license in Ohio.
• Board Certification.
• DEA licensure.
• Clinical background in adult, family or geriatric specialties.
• Experience in long-term care settings.
• Ability to work with an EMR.
• Ability to manage patients with advanced illness and multiple chronic conditions.
• Ability to work in an interdisciplinary care team.
• Psych experience is a plus.

Responsabilidades

We are currently seeking Nurse Practitioners provide medical services to patients living in long term care facilities.

NP ROLE AND RESPONSIBILITIES

• Provide primary and preventative care for long term care patients residing in skilled nursing facilities.
• Manage Residents with acute change in condition.
• Advanced care planning and chronic care management.
• Prescribe appropriate medications, including controlled substances in accordance with OH Law.
• Order, review and/or follow-up with treatments and diagnostic testing.
• Communicate with the nursing home staff on a regular and as needed to discuss changes in condition.
• and/or the care plan.
• Collaborate with primary care physician.

Np Qualifications And Education Requirements
