Job summary

We Care Health Centers is seeking a Nurse Practitioner to provide primary and preventive care to residents in long-term care facilities. You will manage acute changes, perform advanced care planning and chronic care management, and collaborate with the facility staff and primary care physicians.

The role requires an unrestricted OH NP license, board certification, and DEA licensure, with experience in geriatric or adult care and comfort using EMR systems.

Qualifications

• Current, unrestricted NP license in OH.
• Board Certification.

• Clinical background in adult, family or geriatric specialties.
• Experience in long-term care settings.
• Ability to work with an EMR.
• Ability to manage patients with complex chronic conditions.

Responsibilities

We are currently seeking Nurse Practitioners provide medical services to patients living in long term care facilities.

NP ROLE AND RESPONSIBILITIES:

• Provide primary and preventative care for long term care patients residing in skilled nursing facilities.
• Manage Residents with acute change in condition.
• Advanced care planning and chronic care management.
• Prescribe appropriate medications, including controlled substances in accordance with OH Law.
• Order, review and/or follow-up with treatments and diagnostic testing.
• Communicate with the nursing home staff on a regular and as needed to discuss changes in condition.
• and/or the care plan.
• Collaborate with primary care physician.

NP QUALIFICATIONS AND EDUCATION REQUIREMENTS

• Current, unrestricted NP license in the state of OH
• Board Certification.
• DEA licensure.
• Clinical background in adult, family or geriatric specialties.
• Experience working in a long-term care setting.
• Ability to work with an EMR.
• Ability to manage patients with advanced illness and multiple chronic conditions.
• Ability to work within an interdisciplinary care team to coordinate patient care.
• Psych experience a plus
