Job summary

One Medical, a leading primary care provider, is hiring in Phoenix, AZ for a full-time Advanced Practitioner (NP/PA) to manage a diverse patient panel across acute, chronic, and preventive visits.

The role involves in-office and tele-health care, ongoing learning through Clinical Rounds, and collaboration with both in-person and virtual teams. Arizona licensure is required by start date.

Qualifications

• Completed an accredited NP or PA program with a national certification.
• In the past 5 years, practiced as an Advanced Practitioner for at least: 2 years in outpatient primary care, OR 1 year outpatient with a 1-year primary care fellowship or 1+ year in urgent care.
• Arizona state license obtained by start date at One Medical.

Responsibilities

• Manage a patient panel with acute, chronic, and well visits.
• Treat patients in-office, in testing centers, and via tele-health visits.
• Engage in weekly Clinical Rounds and other learning modalities.
