Hearthstone Heath & Rehabilitation in Sparks, NV is seeking a Full Time Physical Therapist for our in-house rehab program. The role involves screening and evaluating patients, delivering skilled rehabilitation, and developing individualized care plans to support geriatric recovery in a post-acute setting.

Qualifications

PT license required.
Open to all experience levels; new grads welcomed.

Responsibilities

Screen and evaluate patients with physical functioning disorders and/or wound care; develop care plans.
Deliver rehabilitative, skilled, and medically necessary treatment interventions.
Administer procedures including manual techniques, ambulation, exercises, modalities, devices, and wound care.
Provide consultation and counseling to patients, families, caregivers, and other providers.
Generate comprehensive discharge summaries meeting regulatory and clinical standards.
