Job summary

Hearthstone Health & Rehabilitation in Sparks, NV is seeking a PRN Occupational Therapist for our in-house rehab program. The role supports post-acute geriatric care with a focus on cognitive/ADL functioning and evidence-based interventions.

You’ll screen patients, develop care plans, provide rehabilitative treatment, document progress, and coordinate discharge planning. OT license required; all experience levels welcome. Flexible schedule and supportive team environment.

Qualifications

• Occupational Therapist license is required.
• This position is open to OTs at all experience levels.

Responsibilities

• Screen/evaluate patients with cognitive and/or ADL disorders and develop care plans.
• Use standardized assessments and evidence-based practice to support interventions.
• Provide rehabilitative, skilled treatment per regulatory requirements.
• Administer ADL retraining, therapeutic activities, exercises, modalities, and assistive devices as part of rehab.
• Assess progress and adjust treatments in the clinical record.
• Evaluate treatment outcomes and refer as needed based on outcomes.
• Provide counseling to patients and families regarding physical disorders.
• Communicate progress and plans with supervisor and health team members.
• Provide comprehensive discharge summaries as required.
