Responsibilities

• Screen/evaluate patients with cognitive or ADL disorders and develop a care plan.
• Use standardized assessments to support interventions.
• Deliver rehabilitative, skilled treatment per regulations.
• Administer ADL retraining, therapeutic activities, exercises, and modalities.
• Monitor progress and adjust treatments in the clinical record.
• Evaluate outcomes and refer as needed.
• Provide patient and family counseling.
• Communicate progress with supervisor and health team.
• Prepare comprehensive discharge summaries.
