BAYADA Home Health Care is seeking a Physical Therapist, PT for a full-time or part-time home health position. The role involves performing home visits for our senior living office in Moorestown, NJ, servicing adult and geriatric clients across Burlington and Camden County.

**Minimum requirements:**
- One year of clinical experience as a licensed Physical Therapist.
- NJ Physical Therapist license.

**Responsibilities:**
1. Make home visits to clients in designated areas.
2. Perform diagnostic tests and measurements, including mobility, strength, balance, and safety assessments.
3. Implement wound care interventions using evidence-based dressings and topical agents.
4. Develop and update individualized care plans, including manual exercises, gait training, and balance interventions.
5. Collaborate with nursing staff on multidisciplinary care plans.
6. Educate clients and families on rehabilitative activities to promote health and independence.
7. Document care activities accurately using electronic medical records.
