Job summary

Springfield Suites Rehab & Nursing Center in Springfield, IL seeks a dedicated Physical Therapist (PT) to provide individualized therapy aimed at improving resident mobility, strength, balance and independence. You will evaluate residents, develop treatment plans, coordinate with PT aides, nursing and physicians, and document progress to support positive clinical outcomes.

Qualifications

• Active PT license required.
• Strong communication and clinical reasoning skills.
• Skilled nursing experience preferred but not required.

Responsibilities

• Conduct physical therapy evaluations and establish treatment plans based on resident needs
• Provide therapy focused on gait training, transfers, strengthening, endurance, mobility, and pain management
• Collaborate with PTA staff, nursing, physicians, and interdisciplinary care teams
• Monitor resident progress and update treatment approaches as appropriate
• Maintain accurate and timely documentation
