Job summary

Wabash Senior Living & Rehabilitation in Carmi, Illinois, is seeking a licensed Physical Therapist to join our in-house rehabilitation team. This full-time position focuses on evaluating residents, developing individualized plans, and helping them regain strength and independence.

You'll collaborate with nursing, physicians, and families to deliver skilled PT services, monitor progress, and ensure compliance with state and federal requirements.

Qualifications

• Current Illinois Physical Therapist license.
• Excellent communication and interpersonal skills.
• Compassionate, resident-focused care approach.
• Team-oriented with interdisciplinary collaboration.
• Experience in skilled nursing or rehab preferred, but not required.
• New graduates welcome to apply.

Responsibilities

• Evaluate residents and develop individualized plans of care.
• Provide skilled PT services based on residents' needs and goals.
• Help residents improve strength, balance, mobility, and independence.
• Monitor progress and adjust treatment plans as needed.
• Collaborate with nursing, physicians, therapy staff, residents and families.
• Complete accurate clinical documentation.
• Maintain compliance with state and federal requirements.
