Job summary

CommuniCare Rehab in Pleasant Valley Healthcare Center, Point Pleasant, WV, is seeking a full-time Physical Therapist (PT). The PT will evaluate patients, develop treatment plans, and work as an in-house team member to provide comprehensive rehab services.

Eligible candidates hold a PT degree (Bachelors/Master’s/Doctorate) with a valid or pending state license. The role includes collaboration with the rehab team, education for patients and families, and potential assignments across multiple

Qualifications

• A Bachelor's, Master’s, or Doctorate Degree in Physical Therapy from an Accredited Program.
• Hold a valid or pending State Physical Therapy License

Responsibilities

• Evaluate, review, and screen patients' medical charts and develop a treatment plan.
• Implement treatment plan and educate the plan of care to the rehab team, supervising PTAs.
• Attend/lead interdisciplinary meetings and train staff/family as appropriate.
