Qualifications

• Licensed Physical Therapist in New York state or eligible.
• Ability to evaluate patients and develop appropriate treatment plans.
• Provide care in line with physician care plans, agency policy and professional standards of care.
• Experience instructing family members, PTAs, Home Health Aides, and other caregivers in physical therapy techniques.
• Mandatory Certifications: First Aid, AED, & CPR Certifications.

Responsibilities

• Deliver personalized PT treatment plans to help patients reach goals.
• Collaborate with a multidisciplinary team in a clinic setting.
• Maintain therapy notes and participate in care coordination.
