Qualifications

• Licensed Physical Therapist in the state of New York or eligible.
• Ability to evaluate a patient’s need for physical therapy and develop an appropriate treatment plan.
• Provide appropriate care and treatment to patients based on physician care plan, agency policy, and professional standards of care.
• Experience instructing family members, PTAs, Home Health Aides, and other caregivers in physical therapy techniques.
• Mandatory Certifications: First Aid, AED, & CPR Certifications.

Responsibilities

• Develop and implement personalized treatment plans to help patients achieve goals.
• Collaborate with a multidisciplinary team to provide comprehensive care.
• Instruct and support caregivers and staff in therapy techniques as needed.
