Qualifications

• Licensed Physical Therapist in the state of New York or eligible.
• Ability to evaluate a patient’s need for physical therapy and develop an appropriate treatment plan.
• Provide appropriate care and treatment to patients based on physician care plan, agency policy and professional standards of care.
• Experience instructing family members, PTAs, Home Health Aides, and other caregivers in appropriate physical therapy techniques.
• Mandatory Certifications: First Aid, AED, & CPR Certifications.

Responsibilities

• Deliver physical therapy to patients through personalized treatment plans.
• Collaborate with a well-trained support staff and use state-of-the-art facilities and equipment.
• Educate patients and caregivers on exercises and home programs to support recovery.
• Maintain accurate documentation and adhere to clinic policies and standards.
