Willow Tree is seeking a caring Physical Therapist to join our skilled nursing team in Topeka, KS. The role includes evaluating patients, delivering rehabilitative therapy, and coordinating with the rehab team to restore function and maximize independence. Qualifications

• Licensed physical therapist with an active state license.
• Minimum 2 years of experience in a skilled nursing facility.
• Bachelor's degree or higher in Physical Therapy or related field.

Responsibilities

• Screen/evaluate patients and develop care plans for cognitive/ADL impairments.
• Use standardized assessments to support interventions.
• Provide medically necessary rehabilitative treatment per standards.
• Administer ADL retraining, exercises, modalities and assistive devices.
• Document progress and adjust treatments in the clinical record.
• Communicate progress with team and families; provide discharge summaries.
