Job summary

Advanced Health Care in Sacramento, CA seeks a Part-Time Occupational Therapist to assess, plan, and implement rehabilitative programs helping residents regain independence in daily activities. You will collaborate with interdisciplinary teams to deliver comprehensive care in our skilled nursing facility.

The role requires California OT licensure, 1+ year of SNF experience, and strong empathy and communication to support residents and families. PRN/Weekdays and Weekend shifts are available.

Qualifications

• Current state licensure as an Occupational Therapist in California is required.
• 1+ years of clinical experience in a skilled nursing facility or similar healthcare setting is required.
• Excellent interpersonal and communication skills with the ability to collaborate with residents, families, and team members.

Responsibilities

• Conduct assessments to evaluate residents’ physical, cognitive, and psychosocial functioning.
• Develop individualized treatment plans based on assessment findings and physician recommendations.
• Provide occupational therapy interventions to improve ADLs and functional abilities, including ADL training, therapeutic exercises, and environmental modifications.
• Document resident progress and discharge plans in accordance with facility policies and regulatory requirements.
• Collaborate with physicians, nurses, PTs, and SLPs to ensure coordinated care.
• Educate residents and families on therapies, safety, and home exercise programs.
• Participate in team meetings and contribute to care planning and goal setting.
