Legend Oaks of North Austin in Austin, TX is a post-acute care facility with long-term and short-term services, seeking a Full Time Occupational Therapist for our in-house rehab program. You will screen and treat patients with cognitive and ADL, using evidence-based practices and standardized assessments to guide care plans.

Qualifications

• Licensed OT with a current license.
• Open to applicants of all experience levels.

Responsibilities

• Screen/evaluate patients with cognitive and/or ADL disorders and develop care plans.
• Utilize standardized assessments and evidence-based practice to support interventions.
• Provide rehabilitative, skilled and medically necessary treatment per regulatory and clinical requirements.
• Administer ADL retraining; therapeutic activities; exercises; modalities; assistive devices as part of the plan.
• Assess progress and adjust treatments in the clinical record per regulations.
• Evaluate treatment outcomes and refer as appropriate.
• Provide consultation to patients, families, caregivers and other providers.
• Communicate with supervisor and health team regarding progress and plans.
• Prepare comprehensive discharge summaries per regulatory requirements.
