Job summary

Enhance Rehabilitation is seeking an Occupational Therapist to join our SNF team in Illinois. You will evaluate and treat patients, develop plans of care, and set measurable, functional goals to support recovery.

You will collaborate with facility staff, provide patient and family education, complete timely documentation, and supervise OT assistants as needed. This role emphasizes delivering excellent patient care and service.

Qualifications

• Evaluating new patients, developing plans of care and setting measurable, functional goals.
• Providing excellent patient care and customer service to patients and families.
• Collaborating with facility staff to ensure high-quality care.
• Completing timely and accurate documentation including evaluations, re-evaluations and discharges.
• Supervising Occupational Therapy Assistants as needed.

Responsibilities

• Evaluating and treating patients in a SNF setting following individualized plans of care.
• Re-evaluating patient progress and revising plans of care to reset goals.
• Providing education to patients and families about injury, progress and outcomes.

• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
• Supervising Occupational Therapy Assistants.
