Job summary

Enhance Rehabilitation is seeking a qualified Occupational Therapist to join our dynamic SNF team. You will evaluate and treat patients in a skilled nursing facility, following an individualized plan of care and collaborating with facility staff to ensure high-quality outcomes.

You will also supervise Occupational Therapy Assistants, document evaluations and progress, and communicate progress and prognosis to patients and families as part of comprehensive care.

Qualifications

• Evaluating patients and developing plans of care with measurable goals.
• Providing patient-centered care and clear education on progress and outcomes.
• Collaborating with facility staff to ensure consistent patient care.
• Documenting evaluations, re-evaluations, discharges and daily progress notes.
• Supervising Occupational Therapy Assistants as part of the team.
